similar to how "you shall not design your own crypto" doesnt apply to the bank: they used well known crypto; similarly individuals could obey that and use RSA and SHA256 pencil-and-paper style, in case they dont know how to design hardware

i.e. from perspective of the bank: they did not design my computer, nor the software I may be running hence they had their my eID smartcard custom designed but uses RSA and SHA256: the hardware, software was designed by them, but not RSA and SHA256

design in my question does not refer to the design of crypto but of the hardware and software: if your processor has builtin sidechannels intended to leak information, or similar with software that is not covered by "you shall not design your own crypto"

Depending on when you consider something a breakthrough I believe this happens much more often than one thinks. Consider the integer overflow on BitCoin: the idea behind the currency is decentralization through open source code of a certain peer to peer protocol. He found a flaw in its implementation. Instead of trying to contact a "certain" centralized person, he showed to everyone listening to the blockchain what the problem was and anyone with debugger skills could find the problem. Also consider that windows virus that closed the vulnerability it used.
