This is actually a modification of the code by jacobsa: github.com/jacobsa/crypto. I didn't bother taking out the added comments, but you are right about why that error was happening in the original code (which is now fixed). Obviously this is not "production-ready" code, but it will get you far enough in terms of understanding the mechanics of the algorithm.

AES-SIV - tools.ietf.org/html/rfc5297 - is an appropriate algorithm to use in this case.

In this case the ctr is initialized with an array of 4 big-endian words, each 4 bytes in length. From what I can tell, this array is used a) to store the L parameter + the IV (up to 12 bytes) and b) to store the counter (4 bytes). Can I just replace the L+IV part with a 12-byte IV, keep the 4-byte counter and do away with all of the rest of the code that deals with authentication?
